# 📝 How to Add a Waiver to the Booking Form

> Add a mandatory waiver or terms of service checkbox to your booking form.

> This is perfect if you want to have your customer accept Terms and Conditions, or possibly have the view and/or sign some external document.

### **Steps to add a waiver to your calendar**

In your Shopify admin dashboard look at the left panel and click on the **Apps.**
Click on **Cowlendar.**
Click on **My Service.**
Click **Edit** on the service you want to add a waiver to.

Click on the **Advanced Tab.**

Scroll down until you see the **Custom Questions section.**
Click on **Add a question.**

Set **Field type to waiver.**
Add a label such as **"Please check the box below if you agree to the terms and conditions".**

Click  **Save.**
**Click Update.**
